Basic and advanced principles of clinical examination, approach to the study of skin disorders and of the medical and surgical therapeutic options in the field of Dermatology.

Learning outcomes and competences

Acquiring knowledge of the clinical pictures, physiopathological mechanisms and principles of therapy for common and/or serious diseases in the field of Dermatology and Venereology.

Training in the observation of the patient focusing on the phototype, general features of integument, mucous membranes, hair and body hair distribution and nails. The morphological examination of the skin lesions considering their type, shape, arrangement and integument distribution will be particularly emphasized as well the recognition of important semiologic signs of skin disease.

Ability to rank the principles of medical decision for a timely medical intervention and referral.

Notions of dermatologic therapeutics, namely topical medications, types of formulations and vehicles (cream, ointment, paste, powder, lotion, solution, suspension, gel, tincture, collodion and aerosol), their effects and indications according to the skin condition, its location, spectrum of inflammation and age of the patient.

Acknowledge of the principles sustaining the prescription of the topical and systemic medications most useful in the treatment of cutaneous disorders, such as costicosteroids, retinoids, antihistamines, vitamin D3 analogs, topical calcineurin inhibitors, cyclosporine, methrotrexate, biologics (monoclonal antibodies and protein fusion molecules), among others. Observation of the main surgical procedures, diagnostic techniques and other therapeutic modalities in Dermatology such as classic surgery, biopsy (punch, shave and excision), curettage, electrosurgery, cryosurgery, laser and intense pulsed light, patch and photopatch testing, prick tests, digital dermoscopy, Tzanck smear, Wood’s lamp examination, diascopy, scraping for fungal detection, histopathological assessment and phototherapy.

Capacity to integrate a team of dermatologists in the outpatient consultation, inpatient ward or surgical unit.

Program

Programme of the theoretical lectures: Dermatological semiology; Bacterial infections of skin and soft tissues; Superficial fungal infections; Viral infections of the skin; Sexually transmitted diseases; Skin manifestations of HIV infection; Tumors and neoplasms of the skin (benign, precancerous lesions and malignant); Cutaneous manifestations of systemic diseases (autoimmune, connective tissue disorders, metastatic skin disease and paraneoplastic skin syndromes); Eczema, urticaria and Psoriasis; Atopic and seborrheic dermatitis.
Seminars: Acne variants and acneiform eruptions; Classification, pathophysiology, clinical presentation and treatment options. Parasitic skin infections; Clinical presentation and treatment of human scabies and pediculosis.
Practical lessons: the practical lessons have the main goal of providing a contact with the outpatient environment that is the main stream of care in Dermatology. A basic training and observation of the patient clinical assessment focusing on the phototype, general features of integument, mucous membranes, hair scalp and body hair distribution and nails will be the main focus. The morphological examination of the skin lesions considering their type, shape, arrangement and integument distribution will be particularly emphasised. Principles sustaining the prescription of the topical and systemic medications most useful in the treatment of cutaneous disorders, such as costicosteroids, retinoids, antihistamines, vitamin D3 analogs, topical calcineurin inhibitors, cyclosporine, methrotrexate and biologics (monoclonal antibodies and protein fusion molecules), among others.
Selected synthetic wound dressings (hydrocellular, hydrogel, hydrocolloid, hydrofiber, activated charcoal silver, alginate, silicone, Unna’s boot, protease-modulating, activated polyacrylate) and their formal indications, in particular in the setting of leg ulcers.
Explanation of the main surgical procedures, diagnostic techniques and other therapeutic modalities in Dermatology such as classic surgery, biopsy (punch, shave and excision), curettage, electrosurgery, cryosurgery, laser and intense pulsed light, patch and photopatch testing, prick tests, digital dermoscopy, Tzanck smear, Wood’s lamp examination, diascopy, scraping for fungal detection, histopathological assessment and phototherapy by PUVA and narrow band UVB.
Hospitalization for severe or extensive skin diseases as an important therapeutic option for adequate skin care, ancillary investigation and improvement of quality of life. Inpatient physical examination and history-taking techniques.

The main assessment of the acquired knowledge is made by a final theoretical test consisting of 20 multiple-choice questions (17 points of a total of 20) and one essay-question (3 points). All students that fulfill the minimal attendance of 75% of all active learning sessions are eligible to the theoretical test in accordance with the requirements in force by the Pedagogic Council of the Faculty.
In addition a 0.85 point bonus will be added to the final mark for those students that simultaneously fulfill the following criteria: a) a minimal assiduity of 80% for theoretical lessions; b) a minimal assiduity of 80% for all learning sessions, including both seminars; c)a score of at least 70% on the practical exam (5 correct answers in 6 questions); d) a score in the theoretical test between 8.5 and 18.5; e) theoretical test in the week where the student attended all the active learning sessions. The practical exam consists in the direct identification of pictures related to dermatological themes covered on the lectures and will take place on the second week of the course, preceding the theoretical exam. An oral exam is possible for the students with a final score between 8.5 and 9.4 and is always compulsory for scores greater than or equal to 19.5.
