Resumo (PT): A obesidade tem sido associada a aumento da androgenicidade na mulher, existindo, contudo, discrepâncias nos resultados encontrados em diferentes trabalhos. Os objectivos deste estudo foram determinar a existência de correlação dos níveis de androgénios ou de SHBG com o índice de massa corporal (IMC), a razão perímetro da cintura/perímetro da anca (Pc/Pa) e a idade, e avaliar a dependência do aumento dos androgénios da presença de obesidade mórbida ou de síndrome metabólica (SM). Foram avaliadas 148 mulheres obesas na primeira Consulta de Obesidade Mórbida do Hospital de S. João, EPE: 105 pré-menopáusicas e 43 pós-menopáusicas. Avaliaram-se variáveis antropométricas, níveis plasmáticos de androgénios (testosterona total – TT, testosterona livre – TL, sulfato de dihidroepiandrosterona – DHEA-S, androstenediona – A) e de globulina de ligação às hormonas sexuais – SHBG. Para determinar a existência de relação linear entre variáveis utilizou-se o coeficiente de correlação e para avaliar a dependência entre variáveis utilizou-se o teste exacto de Fisher. As doentes tinham idade média de 41,8±11,7 anos e IMC de 40,5±7,5Kg/m2. Nas mulheres pré-menopáusicas, o IMC estava negativamente correlacionado com a SHBG (r=- 0,21; p<0,05) e positivamente correlacionado com a TL (r=0,27; p<0,05) e com a androstenodiona (r=0,25; p<0,05). A idade estava correlacionada de forma negativa com a TL (r=-0,26; p<0,05), a TT (r=-0,26; p<0,05), a DHEA-S (r=-0,35; p<0,05), a androstenediona (r=-0,33; p<0,05) e com a razão T/SHBG (r=-0,34; p<0,05). Havia uma correlação positiva entre a Pc/Pa e a DHEA-S (r=0,2; p<0,05). Por seu lado, nas mulheres pós-menopáusicas, apenas se encontrou uma correlação negativa entre o IMC e a SHBG (r=-0,46; p<0,05). Não foi encontrada dependência entre a elevação dos níveis de androgénios e a existência de obesidade mórbida ou síndrome metabólica. Em conclusão, neste estudo encontrou-se correlação quer do IMC, quer da idade com alguns dos androgénios em estudo apenas nas mulheres pré-menopáusicas.
Abstract (EN): Obesity has been associated with increased androgenicity in women, however there are conflicting data on this subject. The objectives of this study were to determine whether any androgen is independently related to body mass index (BMI), waist-to-hip ratio (WHR) or age and to access if there is an association between elevated plasma androgen levels and the presence of either morbid obesity or metabolic syndrome (MS). A total of 148 obese women were evaluated in their first obesity medical appointment at Hospital de São João: 105 premenopausal and 43 postmenopausal. Anthropometric variables, plasma androgen (total testosterone – T, free testosterone – FT, dehydroepiandrosterone sulfate – DHEA-S, androstenedione) and sex hormone binding globulin (SHBG) concentrations were measured. The correlation coefficient and Fisher’s exact test were used, respectively, to determine the strength of the linear relationship and to evaluate the non-random association among variables. The patients had mean age of 41.8±11.7 years and mean BMI of 40.5±7.5Kg/m2. In premenopausal women, BMI was negatively correlated with SHBG (r=-0.21; p<0.05) and positively correlated with FT (r=0.27; p<0.05) and androstenedione (r=0.25; p<0.05). Age was negatively correlated with FT (r=-0.26; p<0.05), T (r=-0.26; p<0.05), DHEA-S (r=-0.35; p<0.05), androstenedione (r=-0.33; p<0.05) and T/SHBG ratio (r=-0.34; p<0.05). There was a positive correlation between WHR and DHEA-S (r=0.2; p<0.05). On the other hand, in postmenopausal women, there was only a negative correlation between BMI and SHBG (r=-0.46; p<0.05). There wasn’t an association between elevated serum androgen levels and the presence of morbid obesity or metabolic syndrome (MS). In conclusion, in this study it was found a correlation between BMI and age with some of the studied androgens, only in premenopausal women.
