Abstract (EN): Psoriasis seems to associate with several comorbidities, namely, type 2 diabetes mellitus, Crohn's disease, hypertension, obesity, metabolic syndrome, cancer, depression and decreased quality of life. An emerging concern is the relationship between psoriasis and the risk for cardiovascular disease (CVD) events.Dyslipidemia, enhanced oxidative stress, increased C-reactive protein, endothelium dysfunction, decreased folate and hyperhomocysteinemia are found with significantly high frequency in psoriatic patients. More recently, other cardiovascular risk factors, like overweight and obesity have been associated with psoriasis. The prevalence of high body mass index (BMI) in psoriatic patients seems to be strongly correlated with increased risk for CVD. BMI may provide, however, surrogate information about CVD risk, as more important than the BMI value is the degree of intra-abdominal fat visceral obesity. The adipose tissue has been recognized as a metabolically active organ, secreting numerous cytokines, which are important in the inflammatory and in the atherosclerotic processes. Aberrant adipocyte metabolism also seems to be associated with psoriasis.The risk for CVD seems to correlate with disease severity, to be more pronounced in patients admitted to hospital for treatment at young age and at several occasions. Indeed, psoriasis per si has been proposed as an independent risk factor for CVD. However, it is unclear whether some of these risk factors, comorbidities and behaviours associated with the disease are related to psoriasis itself and/or to its treatments. Some psoriasis therapies might enhance the cardiovascular risk profile.It has been recognized that patients with immune-mediated inflammatory diseases, as psoriasis, are at higher risk of developing certain comorbidities, namely CVD. Moreover, psoriasis and related comorbidities may share genetic risks, common environmental factors or common inflammatory pathways. Mechanistic links between psoriasis and the related comorbidities may exist. Indeed, inflammation, characterized by an increase in circulating pro-inflammatory factors and endothelial activation, is common to several of these conditions.In summary, according to literature, psoriasis associates with several comorbidities, especially, with CVD. Undoubtedly, a connection between psoriasis and unfavourable cardiovascular risk profile appears to exist. Therefore, it is important to be aware of the associations between psoriasis and those comorbidities and their risk factors, but also to be able to identify potentially treatable situations.
