Abstract (EN):
We performed a prospective study to evaluate the blood sample procedures in the Hospital de Crianças Maria Pia. For a 3 month period, several parameters were registered, namely the number of analytical parameters requested and performed. the number of samples collected and the volume collected and wasted per sample. Such evaluation was applied in newborns and infants hospitalised in the Intensive Care Unit (ICU) and/ or Infants Nursery. In 34 newborns and 8 infants, 1318 samples were collected with a total volume of 568,3 ml. In clinical chemistry samples (n=1091) an excess blood volume of 49,5% was obtained for parameters requiring serum, an excess of 65% was obtained in samples for drugs assays and an excess of 40% in the samples for blood gas analysis. This excess reached 78% for haematological samples (n=163) requiring whole blood. No excess was obtained in the samples (n=64) for Bacteriology. The daily mean iatrogenic loss in 33 newborns hospitalised in ICU was 1,2 ml (0,32 ml - 4,77 ml) and the mean of final cumulative iatrogenic loss was 7,8% of circulating blood volume (0,97 % - 70,6%). Our conclusion is that the hospital blood drawing practice in neonates and infants is amenable to further optimisation in order to reduce the iatrogenic expoliation down to the optimal values mentioned in the literature (25%). Daily and final cumulative iatrogenic losses values are difficult to evaluate in relative terms.
