Abstract (EN): The data available at present suggest certain advantages of intermittent or "pulse" treatment with calcitriol over continuous oral treatment. Patients should have proper phosphate control (serum P < 5.5 to 6.0 mg/dL), and hypercalcemia (serum calcium >10.5 to 11.0) represents a contraindication. Doses of calcitriol above 0.75 ¿g per treatment seem most often associated with adequate suppression of iPTH levels; patients with very severe hyperparathyroidism often require larger doses. The patients must be selected appropriately and the I-PTH level can be a guide to those needing therapy, with serum calcium and phosphate levels to assess "side effects." Continued adequate phosphate control is essential and a reduction of dialysate calcium may permit use of adequate calcium salts as phosphate binders and the needed calcitriol. Yet to be answered are what I-PTH levels should be sought with treatment; some data suggest that the bone features may be suppressed more than the I-PTH levels. Further careful clinical studies are required to answer this and other important questions.
