Resumo (PT):
Introdução: São diversas as barreiras ao cumprimento da terapêutica dietética do excesso
de peso. A desejabilidade social (DS) corresponde à propensão para transmitir uma imagem
culturalmente aceitável, podendo enviesar parâmetros avaliados em investigações
científicas.
Objectivos: (1) Avaliar a DS em mulheres com excesso de peso; (2) Avaliar a associação
entre a DS e outras características; (3) Comparar os níveis de DS entre doentes avaliadas
na primeira consulta ou em consultas subsequentes; (4) Avaliar as diferenças no nível de
DS entre as doentes que referem ou não cada barreira ao cumprimento da terapêutica
dietética; (5) Avaliar o efeito da DS na frequência de cada afirmação indicada como barreira
ao cumprimento da terapêutica dietética.
Material e Métodos: Avaliaram-se 67 mulheres com IMC superior a 25,0 kg/m2 (IMC médio
= 40,0 kg/m2; dp = 5,7) e idade média de 40 anos (dp = 11) quanto à idade, escolaridade,
estatura, pesos actual e desejado, percepção do estado de saúde, peso e imagem corporal
(escala tipo Likert de 1 sente-se muito bem a 5 muito mal). Calcularam-se o IMC, o peso
que deseja perder e o IMC correspondente. Registaram-se o número de consultas prévias
e a data da primeira consulta e calculou-se o tempo desde a primeira consulta. Estudaramse
as características psicopatológicas com a Escala de Bem-Estar Psicológico Geral.
Avaliaram-se as barreiras ao cumprimento da terapêutica dietética por enumeração, de
uma lista de 34 afirmações que referiam obstáculos ao seu cumprimento, daquelas com
que as doentes se identificavam. Avaliou-se a DS recorrendo à Marlowe-Crowne Social
Desirability Scale (MC-SDS).
Resultados: A pontuação média na MC-SDS foi de 20,9 (dp = 4,0). Encontraram-se correlações
positivas fracas entre a DS e o número de consultas prévias e tempo desde a
primeira consulta. As doentes com consultas prévias apresentam níveis de DS significativamente
superiores às avaliadas na primeira consulta (médias de 21,7 e 18,8; p = 0,005).
Não se encontraram diferenças significativas no nível de DS de doentes que indicassem
ou não qualquer uma das afirmações e não se registou efeito significativo da DS na
frequência de qualquer afirmação.
Discussão: A pontuação média na MC-SDS foi superior aos valores encontrados na
maioria dos estudos, o que pode resultar da idade superior, menor escolaridade e presença
de patologia. O contacto prévio com as doentes parece influenciar a DS, podendo
enviesar e dificultar a interpretação de parâmetros medidos por auto-relato. A identificação
de barreiras ao cumprimento da terapêutica dietética parece ser independente da DS.
Abstract (EN):
Introduction: There are several barriers related to the non-accomplishment of the dietary
treatment of overweight. Social desirability (SD), defined as the tendency to transmit
a culturally accepted image, may bias parameters evaluated in scientific studies.
Aims: (1) To evaluate SD in overweight women; (2) To evaluate the association between
SD and other characteristics; (3) To compare the level of SD between patients evaluated
in their first appointment or in subsequent appointments; (4) To evaluate the differences
in the level of SD between patients who mention or not each barrier to the
accomplishment of the dietary treatment; (5) To evaluate the effect of SD in the frequency
of each statement pointed out as a barrier to the accomplishment of the dietary treatment.
Sample and Methods: Sixty-seven women with BMI over 25,0 kg/m2 (mean BMI = 40,0
kg/m2; sd= 5,7) and mean age of 40 years (sd = 11) were evaluated on age, education,
height, present and desired weights, perception of heath condition, weight and body
image (Likert scale from 1 feels very well to 5 very bad). BMI, weight they desired to
loose and corresponding BMI were calculated. The number of previous appointments
and the date of the first appointment were registered and the time since the first
appointment was calculated. Psychopathologic characteristics were studied with the
Psychological General Well-Being Index. Barriers to the accomplishment of the dietary
treatment were evaluated by pointing, from a list of 34 sentences mentioning obstacles
to its accomplishment, those which patients identified themselves with. SD was
evaluated with the Marlowe-Crowne Social Desirability Scale (MC-SDS).
Results: Mean score on the MC-SDS was 20,9 (sd = 4,0). Positive and weak correlations
were found between SD and the number of previous appointments and time since the
first appointment. Patients with previous appointments show significantly higher levels
of SD than those who were evaluated in their first appointment (means of 21,7 and
18,8; p = 0,005). There weren’t found significant differences on the level of SD of
patients who pointed or not each sentence and it wasn’t found a significant effect of
SD in the frequency of any sentence.
Discussion: The mean score on the MC-SDS was higher than values found in the
majority of studies, which may result from the higher age, lower education and the
presence of pathology. Previous contact with the patients seems to influence SD and
may bias and make difficult the interpretation of parameters measured by self-report.
The identification of barriers to the accomplishment of the dietary treatment seems to
be independent from SD.
