Abstract (EN): This review explores a variety of aspects of obesity according to the modern concepts of management. Calculations for determination of obesity degree and the distribution of fat in human body are cited. Types, etiology, prevalence of obesity are mentioned. Leptin, orexins and neuropeptics involved in the ponderal regulation mechanisms and in energy homeostasis are considered. Obesity is considered a chronic disease with multifactorial etiology, including genetics, environmental, metabolic, lifestyle and behavioural components. The preferential and effective treatment of obesity should be based on lifestyle factors such as proper nutrition, regular physical activity, and changes in feeding behaviours. Pharmacoterapic and cirurgic interventions are not excluded for the morbid and for the most resistent types of obesities. Sibutramine and orlistat are the drugs recently approved by FDA which can be tried in pharmaceutical treatments. The epidemic character of obesity and the strong association between obesity and comorbidities, such as coronary heart disease, type 2 diabetes, hypertention and dyslipidemia, recommend to the National Health Service for paying better attention to this disease. Equivalent comparticipation as is given to the other chronic disease should be offered to the obese patients for treatment.
