Objectives

Approach the major methodologies for evaluation of pressures, flows and volumes on the cardiovascular system, including simple evaluation of ventricular pressure and joint assessment of pressure and volume, including the advantages and disadvantages of each of them.

Learning outcomes and competences

Students should have a good knowledge of basic concepts: (i) haemodynamics, (ii) evaluation of pressure and flow, and (iii) catheters and transducers of pressure and flow. Know (i) methodologies for invasive and non-invasive assessment, (ii) experimental preparations with and without anesthesia, (iii) experimental preparations with open and closed chest, and (iv) influence of ventricular interaction and neuroendocrine systems. Know the main methodologies for assessment of ventricular function in vivo, as well as the indices of systolic and diastolic function. Understand the representation of the cardiac cycle as pressure-volume loop and understand the roles played by pre-load, after-load, contractility and compliance. Understand the characterization of ventricular systolic and diastolic properties from end-systolic and end-diastolic pressure-volume relationships. Application of concepts to clinical practice and scientific literature.

Basics of haemodynamics. Pressure and flow assessment: techniques and methodologies. Catheters and transducers for pressure and flow measurement: techniques and methodologies. Methodologies for evaluating ventricular volumes in real time. Systolic and diastolic function indices obtained by analysis of pressure. Indexes of relaxation: methodologies for calculation of the isovolumetric relaxation time constant (tau). Dependence of load. Joint analysis of variations of pressure and ventricular volume throughout the cardiac cycle. Transient load manipulation to obtain robust and independent indices of contractility and filling capacity. Changes in cardiovascular pathology. Invasive and non-invasive evaluation. Experimental preparations with and without anaesthesia. Anaesthesia for invasive assessment methods and its haemodynamic effects. Experimental preparations with open and closed chest. Analysis of clinical cases and literature.
