Objectives

Oral health is fundamental for the overall health of each patient and, as in other medical areas, also in dentistry, a holistic approach to the patient is essential, considering his biopsychosocial condition, other pathologies that he presents, the usual medication and possible allergies, that allow choosing the most appropriate therapeutic approach for the individual.

The Curricular Unit of Medical Specialties I has the following general objectives for its students:

- The acquisition of postures and attributes essential to the performance of the profession and the establishment of an adequate doctor-patient relationship;

- Knowing how to stratify the patient into risk groups and choose, based on that risk, the most appropriate place and treatment, according to the data collected in the anamnesis;

- Knowing how to recognize the main symptoms and signs of the pathologies addressed and the implications for the practice of dentistry, namely the recognition of the need for further referral

- Knowing how to act in the most common urgent clinical situations in the field of dentistry.

Learning outcomes and competences

In this Curricular Unit, students are expected to acquire the following skills:
- The ability to collect and record the patients’ clinical history;
- Understanding the relevance of the pathologies addressed and the implications in dentistry of existent systemic disease and its treatments;
- The ability to individualize treatment according to the patient's pathologies, their usual medication and possible allergies;
- The ability to act in the face of a medical emergency that occurs in the dentist office.
- Knowledge of the most common clinical manifestations of chronic pathologies such as:
* heart failure, dysrhythmias and syncope
* respiratory disease
* liver disease
* kidney disease
* diabetes mellitus

- Knowledge of when and how to do antibiotic prophylaxis
- Knowledge of hemostasis alterations and management of hypocoagulated patients
- Ability to diagnose and treat the main pathologies of the oropharynx
- Knowledge of the main pathologies of the maxillofacial area
- Knowledge of the metabolism of the drugs most frequently used in the practice of dentistry, their toxicity, drug interactions and the need for dosage adjustment
- Ability to manage common medical emergencies at dentistry office

Theoretical classes
Class 1 – Biopsychosocial approach to the patient. Clinical interview. Communication. Signs and symptoms of illness
Class 2 – The patient with heart failure
Class 3 – Dysrhythmias and Syncope
Class 4 – Common manifestations of chronic respiratory diseases
Class 5 – Oral health and relationship with heart and lung disease. Antibiotic prophylaxis: when and how to do it
Class 6 – Diseases of the oropharynx
Class 7 - Changes in hemostasis
Class 8 - Management of hypocoagulated patients
Class 9 - Diabetes mellitus
Class 10 – Chronic liver disease: clinical manifestations
Class 11 – Chronic kidney disease, nephrotoxicity and drug adjustment
Class 12 - Polypharmacy and drug interactions
Class 13 – Maxillofacial Pathology I
Class 14 - Maxillofacial Pathology II 

Theoretical-practical classes
1. Clinical interview – role playing
2. Physical examination: vital parameters (training station at the Biomedical Simulation Center)
3. Discussion of clinical cases of the topics covered in theoretical classes:
* Decompensated heart failure 

* Asthma 

* endocarditis 

* Chest pain 

* Abuse of NSAIDs 

* Hypoglycemia 

* Patients with hypocoagulation and hemorrhage 

* syncope 

* Alcoholism and aspiration pneumonia 

* hypertensive crisis

Teaching methods and learning activities

Theoretical course: plenary theoretical classes lasting 50 minutes/week. Approach to the contents identified above
Theoretical-practical course: theoretical-practical classes (2h/week) with hands-on stations and discussion of clinical cases
Portfolio: 2 assignments on topics relevant to dentistry professional practice

Eligibility for exams

According to the General Regulation for Student Assessment of the University of Porto, a student is considered to have attended a course unit if, having been regularly enrolled, he or she does not exceed the limit number of absences corresponding to 25% of the scheduled classes, as per regulation existing in the organic unit.

Calculation formula of final grade

Final classification (scale 0-20)
- Continuous assessment (25% of the final assessment)
- Portfolio (25% of the final assessment)
- Final exam: Multiple choice test with 60 questions for 90 minutes (50% of the final assessment)

Classification improvement

The improvement of the classification of the final exam will be carried out in the written test that takes place in the two following exam seasons, as foreseen in the regulation of the UP.
