Abstract (EN): Background: Echocardiography plays a pivotal role in detecting hypertrophic cardiomyopathy (HCM) and understanding its pathophysiology. However, the heterogeneous distribution of hypertrophy in HCM limits the accuracy of echocardiographic conventional measurements in the evaluation of left ventricle hypertrophy (LVH). The echocardiographic Spirito-Maron score (SMS) has been described as an accurate index of LVH, in HCM patients. The aim of this study was to assess the correlation of SMS with left ventricular (LV) mass quantification by Cardiac Magnetic Resonance (CMR). Furthermore we evaluated the correlation of SMS with echocardiographic parameters of LV diastolic function. Methods: We performed two-dimensional (2D) transthoracic echocardiography in 39 patients with HCM, and evaluated LV diastolic function parameters, SMS, LV mass (ASE recommended formula) and maximal wall thickness (MWT). The SMS was obtained by adding the MWT of four LV segments, at basal or midventricular level, in short axis view. CMR was performed in 27 of these patients. The echocardiographic parameters of LVH, including SMS, were correlated with LV mass obtained by CMR and with E/e′ ratio. Results: Of the 39 patients included 23 (59%) were male, age 48 ± 18 (range 18-76). We found a highly positive correlation between SMS and CMR LV mass (r=0.817; P<0.001), while MWT (r=0.583; p=0.001) and the 2D-ventricular mass (r=0.429; p=0.026) presented a lower correlation coefficient with CMR LV mass. The SMS was significantly correlated with E/e′ ratio (r=0,655; p<0.001), while a lower correlation was found with MWT (r=0.458; p=0.004) and 2D LV mass (r=0.394; p=0,003). Conclusion: SMS presents a highly positive correlation with CMR LV mass and with diastolic dysfunction severity in CMH patients. SMS is a reliable quantitative LVH measurement method and provides a more comprehensive morphological and physiological information than 2D-echocardiographic conventional parameters used to estimate LVH.
