Abstract (EN):
Introduction: Secondary tumors are much more common than primary tumors, however, the cardiac metastasis of laryngeal carcinoma is uncommon.
Case report: We describe the case of a man of 71 years old, hypertensive, ex-smoker. One year ago he was diagnosed with epidermal carcinoma of the larynx, having undergone total laryngectomy and tracheostomy. Admitted to the Emergency Service with 2-week evolution of symptoms suggestive of respiratory infection. Analytically, with elevated inflammatory markers. The chest radiography showed pleural effusion and bilateral pulmonary infiltrate, having been admitted with the diagnosis of community-acquired pneumonia. Empirical antibiotic treatment was initiated, later guided by antibiogram. In the absence of therapeutic response and progressive clinical deterioration, a transthoracic echocardiogram was performed and revealed a large infiltrating mass within the right ventricle, involving the apex, interventricular septum and free wall, not conditioning significant right ventricular outflow tract obstruction. He also had preserved biventricular systolic function, without evidence of vegetations. The cardiac mass observed was interpreted as a secondary tumor of the laryngeal cancer. For further evaluation and staging of the neoplastic disease, abdominal-cervical-thoracic CT scan was performed, which showed signs of lung, bone, liver, adrenal and heart metastisation. At the heart, an injury also stood out at the dependence of the pericardium adjacent to the left ventricle. The patient was considered for palliative treatment, given the end stage of the neoplastic disease, of poor prognosis. He is currently at home, in superimposable clinical status, maintaining antibiotic treatment.
Conclusion: Late diagnosis of metastatic cardiac tumors often implies high rates of morbi-mortality, as well as extensive spread of the disease, with multiorgan involvement. Early imaging characterization plays an important role for a reasoned decision and appropriate therapeutic approach.
