Buerger’s Disease (tromboangiitis obliterans) is a specific entity, of nonatherosclerotic, yet unknown etiology. It’s a chronic, progressive disease, dependent upon smoking habits, affecting small and mediumsized limb vessels. In this work we present a series of 207 patients (199 male, 8 female). Major amputation occurred mainly during the 1st decade of follow-up. A study was conducted at Hospital São João, comparing sympathetic function between Buerger’s Disease patients and healthy controls. Simpathectomized patients had diminished levels of nor-adrenalin, proportionally to the length of post operative follow-up. The most important therapeutic measure is smoking cessation. Lumbar simpathectomy was performed in most patients with a trophic lesion. Direct surgery was performed in 11% of cases. Major amputation was avoided in more than 90% of patients.
