Chronic rhinosinusitis, currently considered a public health problem, is a heterogeneous group of entities with a multifacto-rial etiology and is characterized by the presence of characteristic nasosinusal symptoms in association with clinical or radiogra-phic features of inflammation. It can be subdivided into chronic rhinosinusitis without nasal polyps (CRSsNP) and chronic rhino-sinusitis with nasal polyps (CRSwNP), with several variants that can be considered as additional chronic rhinosinusitis subphenotypes. It is also important to take into consideration its main differential diagnosis. The preferred medical treatment should be directed towards nasosinusal inflammation. Surgical treatment might be necessary in patients who are refractory or present contraindi-cations to medical treatment. This paper aims to review the literature on chronic rhinosinusitis presenting a clinical vignette of a patient with CRSwNP followed in an Allergy and Clinical Immunology Department as a starting point. © 2022, Sociedade Portuguesa de Alergologia e Imunologia Clinica.
