Abstract (EN): Eyelid surgery must establish and maintain the integrity of the lid margin. A variety of techniques are available to reconstruct full-thickness large defects of the eyelid margin. Hughes tarsoconjunctival advancement flap supplies the deficient lower lid with conjunctiva for lining and tarsus for structural support, and results in a reconstructed eyelid of excellent colour match, thickness and function. In naevoid basal-cell carcinoma syndrome, a rare autosomal dominant skin disease, multiple basal-cell carcinomas begin to appear in the first or second decades of life on both sun exposed and non-exposed areas. The major complication of the disease is the enormous number of basal-cell carcinoma lesions and their optimal management. A 36-year-old Caucasian female with naevoid basal-cell carcinoma syndrome was admitted for a nodular ulcerated lesion occupying the right lower eyelid, clinically suggestive of basal-cell carcinoma. The therapeutic approach involved surgical excision of the lesion and reconstruction of the lower eyelid with the Hughes procedure, both from the upper eyelid. The surgical margins were clear and there was a good cosmetic and functional result.
